Staff Forms and Documents
Staff Position Application Forms
Carefully read these instructions, and the instructions of each form before you begin to fill out the application. The application(s) must be filled out in its entirety before submission. There are three staff position applications—1-Tutoring, 2-Performing Arts and 3-General Applicants.
*All applicants will undergo background checks, except applicants under the age of 18.
Tutoring Application
1. Enter all your information on the application and fill in each section to the best of your ability.
2. You must submit a total of three references which may include previous employers, volunteer supervisors, relative, and/or any professional relationship that can provide insight to who you are and your work ethic.
3. You must initial and sign the acknowledgement and authorization portion of the application.
Performing Arts Instructor Application
1. Enter all your information on the application and fill in each section to the best of your ability.
2. You must submit a total of three references which may include previous employers, volunteer supervisors, relative, and/or any professional relationship that can provide insight to who you are and your work ethic.
3. You must initial and sign the acknowledgement and authorization portion of the application.
General Application
(For applicants under the age of 18)
1. Enter all your information on the application and fill in each section to the best of your ability.
2. You must submit a total of three references who may include previous employers, volunteer supervisors, relative, and/or any professional relationship that can provide insight to who you are and your work ethic. *Applicants under the age of 18 are not required to submit employment reference.
3. You must initial and sign the acknowledgement and authorization portion of the application.

Is AIEP Open?
We are closed to the public for in-person assistance. However, we are providing virtual (phone, email) support to families from Monday to Friday, 12pm to 6pm.

I'm new to OSI AIEP. How Do I Enroll in the Program?
Families can apply to our program using our Registration Form.
You can also call 888-888-8888 or set up an appointment with a counselor online. Our team will guide you through the process.
How Do I Submit My Forms to AIEP?
All in-person services are suspended at AIEP. However, families can continue to submit forms online to AIEP.

How Do I Change My Address, Phone Number, or Other Information?
You can change your home address, phone numbers, etc., by completing our online Information Change Form.
